Specialty Care

Some community members may have certain conditions or find themselves in situations that require special attention and care. To assist and support in these cases, Advena Living at Fountainview offers a range of programs and services that include sepsis and infection management, wound care, and treatment of pulmonary conditions.

We also offer services for palliative care/hospice in cases of advanced disease, and we facilitate the delivery of social services from licensed social workers.

Sepsis Management Program

Sepsis is a potentially life-threatening condition that occurs when the body has an extreme response to an infection that damages its own tissues. Because sepsis can be especially serious for the senior population, Advena Living at Fountainview has developed a Sepsis/ID Management Program to promote early identification, provide treatment and reduce re-hospitalizations related to sepsis:

Clinical staff specially trained in the treatment and early identification of infections and sepsis (TMF curriculum)
Stat lab capabilities for CBCs and blood cultures
Antibiotics adjusted with culture and sensitivity results
Standing orders for initiation of hydration protocols via IV fluids
IV antibiotic administration (urgent and routine)
Daily labs until cleared
Therapy initiated as soon as possible to ensure quick recovery

A proactive treatment option included in our sepsis program is Theraworx Protect, an integral part of infection prevention and control programs for reducing healthcare-acquired conditions, hospital readmissions and antibiotic use. It is applied to the skin to promote skin and urinary tract health.

We also provide management and treatment of other infections that are secondary to:

Pneumonia
Wounds
Osteomyelitis
Drug resistance

Pulmonary Medical Recovery Program

The goal of our Pulmonary Medical Recovery Program is to help our community members breathe easier and their caregivers rest easier. Advena Living at Fountainview specializes in the treatment and recovery of complex pulmonary conditions, using modalities such as:

Tracheostomy care/weaning/decannulation
Oxygen therapy (BiPAP and CPAP)
Nebulizer treatment
Pulmonary strengthening and recovery

We utilize patient-centered clinical pathways and follow the recommendations of the American Association of Respiratory Care and the INTERACT 4.0 program. This enables our community members to get back home faster and prevents unnecessary return trips to the hospital.

Our program focuses on chronic and acute conditions, including:

PneumoniaCOPD (chronic obstructive pulmonary disease)
CHF (congestive heart failure)
ALS (amyotrophic lateral sclerosis)
Asthma
Post-operative care

Wound Care

Advena Living at Fountainview partners with Wound Care Plus to provide comprehensive wound care on-site. Our community members can receive surgical debridement, skin lesion biopsies and abscess drainage procedures at bedside, so they can likely avoid a transfer to a hospital for treatment.

We offer treatment and assistance for the following:

Pressure wounds
Diabetic wounds
Vascular wounds
Post-operative wounds
Unna boot (compression dressing)
VAC (vacuum-assisted closure) therapy

Palliative Care and Hospice

Our palliative care services include a comprehensive, multidisciplinary care program for community members with severe illnesses. Palliative care focuses on relieving pain and other symptoms at any stage of serious disease or diagnosis. Our team aims to improve the quality of life and provide the utmost support for community members and their families during this challenging time.

In the case of advanced disease, it is our honor to ensure that our community members continue to live with quality and dignity. Caretakers and family members can feel secure in knowing that the final days of their loved ones are filled with comfort, meaning, and compassionate care in a homelike setting.

Social Services

An important component of the therapeutic services offered at Advena Living at Fountainview is the delivery of psychosocial services to our community members and their families.

The Social Services Department, which delivers these services, is composed of professionally licensed social workers with experience in the field of gerontology and long-term care programs. The array of numerous psychosocial services includes advocacy, service coordination, discharge planning and supportive intervention.
